LIABILITY WAIVER:

I Name Submitted Below (under 18, parent or guardian) understand that an injury may result from participation in any Nuthin but Net event activities. I hereby release, Nuthin but Net, LLC, Terrence Bridgeforth, Camp Awesome,Athletic Trainers, Event Staff and Event Location and any of their emissaries from any and all liability, claims, demands, action and causes of action whatsoever arising out of or related to any loss, psychological, emotional or economical damages, illness, physical injury, including death, that may be sustained by the participant or my child while participating in such event related activities, or while in, on, or upon the premises where the event activity is being conducted. As the participant or the parent/guardian of the participant, I also give permission for any event transportation, medical treatment, which may be advised or recommended by athletic trainers on site, emergency medical care or treatment that may be required, including medical transportation and accept responsibility for the costs. I understand that I am financially responsible for any medical bills incurred by my child(ren) while participating in events.

By signing this waiver, I also authorize the use of pictures of the below-named participant to be posted on the Nuthin But Net web site or advertising media published by Nuthin But Net.

In case of an emergency, I authorize Nuthin But Net Basketball Camp staff and/ or the Faith Lutheran School staff to administer first aid to my child and/or call 911 to get emergency personnel for further treatment.

LIABILITY WAIVER

LIABILITY WAIVER: I am aware that participation in the Nuthin But Net Basketball Camp has some inherent risks and injury can occur. On rare occasions these injuries can be serious. In consideration of my child being allowed to participate in the Nuthin But Net Basketball Camp, I, the parent/guardian, assume the risk of all injury and agree not to sue Nuthin But Net, the camp directors, coaches, assistant coaches, agents, or volunteers for any and all injuries caused by or resulting from participating in the Nuthin But Net Basketball Camp. I in consideration for allowing my child to participate in the Nuthin But Net Basketball Camp hereby forever release, Nuthin But Net Basketball Camp and Faith Lutheran School and any of their emissaries from all liability for any and all damages and injuries suffered by my child while under the instruction, supervision, or control of any of the above mentioned parties. As legal guardian of the aforementioned person, I hereby agree to individually provide for any possible future medical expenses which may arise as a result of my child training, participating in activities or competing in games for the Nuthin But Net Basketball Camp. I also agree not to bring legal action against Faith Lutheran School and any of their emissaries. By signing this waiver, I also authorize the use of pictures of the below-named participant to be posted on the Nuthin But Net web site or advertising media published by Nuthin But Net.
